Please check the boxes as attachments are added:
- High School Transcript
- Copy of ACT Results
- Listing of Awards and Club Memberships
- Letter of Admission from the school you will be attending
- Character recommendation from two adults (other than family members) who know you well
- Parent’s 1040 Tax Statement from the immediate prior year.
Note: No applications will be accepted after April 20th. Every student must submit this entire application and sign the agreement before being admitted as a candidate into the scholarship program.
